Usually the person seeking the services is the one asking the questions.

Here's the most important question to ask a facility: "Do you have Medicaid beds?"

To be honest, I cannot find anything about these "five questions" in determining whether or not ECF is needed.
However, to my mind as an 82 year old retired RN there is only ONE QUESTION:

IS my ELDER now SAFE alone at home?

This question brings to the fore all sorts of OTHER questions:
1. Is there dementia? That one means ECF is needed or 24/7 home care (not affordable unless we are talking multiple millions of dollars.
2. Are there severe disabilities in mobility due to illness, injury or balance.
3. Are executive functions so impaired that the individual cannot function to shop, cook, eat safely.
4. Are there issues of incontinence and inability to maintain a clean and safe environment.

I don't think there is anyone out there who is an adult, seeing the failure in a loved one, who does not KNOW when it is unsafe.
With that comes the "risk assessment" and the willingness to allow to this adult to actually DIE in home when they might LIVE a bit longer with care.

There are many things involved, here, Charles, and you might get better answers if you tell us your circumstances and the issues you are weighing. Wish you luck.
